Вследствие острой задержки мочи происходят серьёзные изменения не только в мочевом пузыре, уретре, но и в других внутренних органах. Уровень смертности в результате острой задержки мочи достигает 8,5%. Исследования проведены на 18 котах, которым была выполнена перинеальная уретростомия с удалением полового члена. У 2-х котов половой член удалялся полностью, у 16-ти котов оставлялась культя полового члена длиной около 2 см. За всеми пациентами после операции велось наблюдение в течение от 6 месяцев до 5 лет. Диагноз ставился на основании данных анамнеза, клинического осмотра, результатов лабораторных исследований, методов инструментальной диагностики. Показаниями к выполнению перинеальной уретростомии в 56% клинических случаев была рецидивирующая обструкция уретры, в 33% клинических случаев - первичная обструкция уретры уролитами или уретральными пробками и в 11% случаев показанием была травма полового члена (самоиндуцированная животным и ятрогенная). За всеми прооперированными животными велось клиническое наблюдение. У 44% животных после проведения операции не наблюдалось каких-либо осложнений, связанных с операцией, и рецидивов основного заболевания. У 56% животных за период исследования наблюдались следующие осложнения. травмирование стомы, недержание мочи, стеноз уретры, повторная обструкция уретры, зарастание стомы. За всеми прооперированными животными велось клиническое наблюдение. Наиболее часто встречающееся осложнение операции (35% осложнений) - это зарастание стомы. Часто зарастание стомы сочетается со стенозом уретры (25% осложнений). Проведенные исследования показали, что перинеальная уретростомия не является операцией первого выбора. Выполнять перинеальную уретростомию с удалением полового члена следует только в тех случаях, когда нарушена проходимость удовой части уретры и/или дистального отдела тазовой части уретры.</p></abstract><trans-abstract xml:lang="en"><p>One of the manifestations of diseases of urinary excretory organs is the acute urinary retention that is a life-threatening pathological condition in cats. The acute urinary retention leads to the pathological changes not only of the bladder and the urethra, but also of the other internal organs. As a result of the acute urinary retention the mortality rate can reach 8.5%. Studies were carried out on 18 cats that have undergone the perineal urethrostomy with removal of the penis. The penis was removed completely in 2 cats, a stump about 2 cm of a length was left in 16 cats. All patients after the operation were followed for 6 months to 5 years. The diagnosis was made using the data of anamnesis, the clinical examination, the results of laboratory analysis, and also instrumental diagnostics. Indications for performing the perineal urethrostomy for 56% of clinical cases were recurrent urethral obstruction, for 33% of clinical cases - primary urethral obstruction by uroliths or urethral plugs and for 11% of cases - a penile trauma (selfinduced and iatrogenic). The clinical observation was carried out for all operated animals. There were no complications associated with the operation and relapses of the underlying disease in 44% of the animals after the operation. The following complications were observed during the study period in 56% of animals: trauma of the stoma, urinary incontinence, urethral stenosis, repeated obstruction of the urethra, overgrowing of the stoma. The clinical observation was carried out for all operated animals. The most common complication of surgery (35% of complications) is the overgrowth of the stoma. The overgrowth of the stoma often is combined with urethral stenosis (25% of complications). Studies have shown that the perineal urethrostomy is not an operation of the first choice.
